Vent: Saw a new build in Cedar Rapids with the flue pipe just shoved into the chimney, no proper collar or seal. Looked like a real fire hazard.

Yikes, that's scary to hear. Was this on a house that people had already moved into? I always wonder who signs off on this stuff during the final inspection. Seems like a huge red flag that should have been caught.
