Just caught someone at the plant using a chipping hammer with a mushroomed head

Walked past bay 3 yesterday and saw Joe hammering away with a chipping head that was all flared out. Told him to stop and grab a new one. That mushroomed steel can fly off and hit you in the face. He said he didn't notice it was that bad. Anyone else keep an eye on your crew's tools or just assume they check themselves?

That mushroomed head on bay 3 is exactly why I tell my guys to trade in any tool that looks questionable, no questions asked. @evan151 I get what you're saying about training, but we can't assume every guy will notice a hairline crack or a flared edge when they're in the middle of a shift. A quick walk-around catches things before they become a trip to the ER.

Three times last year I had to pull guys off the floor for using tools with mushroomed heads and cracked shafts. Maybe if we spent less time walking around like hall monitors and more time actually training guys on what to look for, they'd spot it themselves. Seems like a management control issue dressed up as a safety concern.
