A 20 year old Schindler 330A in a Boston office tower just ate a set of my good brushes. The governor rope frayed and wrapped around the sheave in a way I've never seen before.

Honestly, that sounds like a maintenance issue, not the elevator's fault. A twenty year old part is bound to fail if it wasn't checked properly. Maybe those brushes were just in the wrong place at the wrong time.

Maintenance issues can still be serious though... a frayed governor rope isn't just a worn out part. That's a key safety piece that should have been caught long before it got that bad. Makes you wonder what else they missed on the inspection. Calling it "wrong place, wrong time" kinda downplays a real system failure.
