The bleach mishap that made me double check every formula since

Back in 2017 at a salon in Tacoma, I mixed a toner wrong and a client's hair turned a weird shade of green right before her wedding trial. I had to strip it and redo the whole process in 90 minutes, which taught me to always write down ratios even if I've done it 200 times. Has anyone else had a color result that forced you to change your whole process?
