Spindle crash from a 50 cent washer last Thursday

Was running a production job on a Haas VF-2 at my shop in Cleveland. Didnt notice a tiny washer had fallen onto the T-slot under the part. When the table moved for the next operation it lifted the part about .015 inches. The tool hit the part, snapped the insert, and gouged the finish face. Now I sweep every single fixture before every single M06. Anyone else had a crash from something that small?

Wait, wouldn't a .015 lift just chip your tool, not snap it?

Has anyone checked if the collet might've been bottoming out before the tool even hit the cut, which could explain both the chip and the snap at that tiny lift?
