Plunge router bit walked sideways on red oak, now I'm doubting my setup
Was routing a hinge mortise on a solid core door last Tuesday and the bit drifted about a quarter inch before I caught it, so I checked the collet and base plate and found the whole assembly had loosened from vibration, has anyone else had a router base work itself loose mid-cut on dense hardwoods?

Honestly sounds like you just didn't torque it down enough. I've run the same setup on white oak for years without a single hiccup. Vibration is part of the deal, you gotta check those screws like they owe you money.
