The referral bonus that cost me my reputation

I sent a buddy to an open role at my company in Chicago, promised the $1,500 bonus would be easy. He quit after 2 weeks and my manager asked why I vouched for him. Has anyone else had a referral blow up on them like this?

Ten years ago I vouched for my wife's cousin at the factory, and he got caught sleeping in the break room on his third day. My supervisor still brings it up at Christmas parties, so I feel your pain.
