I was using the wrong crimp tool for Cat6 for a full year before a client in Tacoma pointed it out.
He asked why the jacket wasn't fully seated in the plug, and I realized my tool was for a different style. Anyone else have a goof like that they only caught because a customer noticed?

Oh man, the wrong crimp tool... I did the same thing with a punch down tool for a month before a coworker asked why my terminations looked so weird.

Classic new guy move. The worst part is the slow dawning horror when you finally see a proper crimp. Makes you question every cable you've ever touched.
